OZH Free Sports Physicals Clinic

Dear Parent(s) of Student Athletes:

Any student planning to attend a sports camp or participate in any sport for the upcoming 2025-2026 school year must have a sports physical on file PRIOR to participation. 

Sports physicals will be performed at Richards School on Wednesday, May 14 in partnership with Ozarks Healthcare. The sports physicals will be performed during the school day and the opportunity will be available to students enrolled in Summer School. 

 All students will be required to submit a signed consent form from a parent or legal guardian prior to participation. Sports physical forms will be sent home on the first day of Summer School, Monday May 5th and are available at the front office and on the school's website. 

Students not enrolled in Summer School can obtain a sports physical free-of-charge at the OZH Zizzer Clinic from June 16 to June 27 or can, alternatively, schedule an appointment with their primary care physician.
