OHC Free Sports Physicals Clinic

Dear Parent(s) of Student Athletes:

Any student planning to attend a sports camp or participate in any sport for the upcoming 2023-2024 school year must have a sports physical on file PRIOR to participation. 

Students can obtain a sports physical at their primary care physician or at the upcoming sports physical clinic offered free-of-charge by Ozarks Healthcare on May 23, 2023.

Sports physical forms are available at the front office and on the school's website.

The physicals will take place at the Ozarks Healthcare Orthopedics and Spine located on the second floor at 1100 Kentucky Avenue.

OMC will offer FREE athletic physicals to student athletes. All students will be required to submit a signed consent form and provide current insurance information from a parent or legal guardian. Consent forms may be picked up at the main office at Richards School and on the school's website.

Students should arrive between 4:00 and 7:00 p.m.
